Jamil is very smart, and has no academic deficits at this time. The easiest way to get him motivated to socialize is surrounding academic activities. Attached is a list of things Jamil knows, and he has had no problems generalizing skills thus far. He loves music, and instruments, at home he has a full size keyboard, drums, guitar, xylophone, and many single drums, as well as some flutes, harmonicas, and bells. He finds these items very relaxing, and if the school does not have any of these items I would like to donate some to the class room because I think this has helped Jamil’s learning tremendously. He knows many nursery rhymes and enjoys singing them (but gets upset if people join him), Jamil knows many hand gestures to songs, but generally is very bothered if other people sing (besides myself, and even for me, he only wants me to sing certain songs) If other people sing he will often request them to stop (“I don’t want to singing”, or “I don’t like it singing”) He associates different songs with different activities, so if he is doing a activity, he will request a specific song (certain swings go with certain songs, one specifically is the “baby bumble bee swing” This swing is generally called a Taco Swing by OT. He expects to go inside on his back, and be vigorously rocked back and fourth singing the baby bumble song. This activity is very calming for him. We had him on a Brushing protocol and saw a lot of success, but several months ago he started rejecting the brush, this is something I would like to start up again, and think would benefit him.

Jamil’s biggest deficits are Communication, Joint Attention, Eye contact, and Peer play. His social skills are minimal, and prompted. He has a hard time with responding to open ended questions, and will avoid you when you ask a question or make a statement that is aversive (ex. Lets to the store, he will actively seek an activity and avoid you, until you go to put his shoes on, he will start to cry. Or when given a choice “Do you want go with Mommy or stay home with Daddy?”, he wont respond to the question until you’re ready to leave, he will then cry he wants to stay home and fight going into the car seat.) We have seen a huge increase in Eye contact recently, but it is still far less than average. He plays okay with older children, but will complain constantly about same age peers. He makes statements like (“Quinny’s bothering me, Quinny’s whining, I have a headache etc…)
















Jamil has become obsessed with grouping, lining, and sorting things, and this has gotten in the way of performing some of the standard tests, (example, if given letters that are all different colors, he wont mix the colors to make a word, or he went through a period that blocks were aversive because when he went to stack them they sometimes fell over thus breaking the group, we have had a hard time shopping, because if he sees a poster of a spill, he can’t get over that it “made a mess” and will continue to talk about this for a while after) So if you find he wont complete a task, it is most likely not that he can’t, but that he won’t. I have found it is easiest when you find a problem to just change the materials, and ask a different way, but sometimes he can quickly perform the task, if he is allowed to manipulate the materials afterwards. It also gets in the way of peer play, because he becomes very distressed if a child removes one item from a group, and he will try to get the item back, or he will cry, because his experience has shown him that removal of the item will lead to another child crying. Standard tests are often easier using 2D materials, he knows both the same, but when items are put into the field, he wants to put them in a pile, and wont respond to the question. When highly motivated, he can over come much of the rigidness, and he understands contingency.

Jamil loves animals and figurines, but toys like this often cause a problem when playing in a group, or if one animal is missing.. (If given a cow and a pig, he will be upset looking for a sheep and horse) Toys generally go in pairs or groups, and he will always seek a third figurine, unless it’s a pair… (Two Elmo’s, or two horses) This is something we have been working on a lot at home, and with his current therapy, we have made a lot of progress under controlled settings, but if he is distressed his need for groupingincreases. He will pretend play while he is i


Jamil’s imitation skills are very strong. He will imitate hand gestures, sound patterns, or play sequences. He has never had a problem in this area. He also has good turn taking skills with an adult (and sometimes peers, but this skill is highly prompted) with games like Break the Ice, Cariboo, Picture Bingo, Coloring, Patty Cake, or other highly structured activities. His Receptive Instruction skills are very good too, he can perform upwards of 4-chain instructions, especially surrounding musical instruments. An example of this would be “Play the Guitar, Big drum, little tambourine, then give Daddy a kiss” Or “Put Barney on the Circle, and the cow on the square, then hit the drum” He loves playing games like this and will request for you to give him instructions while he plays instruments.

Jamil has some very extreme sensory issues. He often becomes very distressed when undressed. I would prefer to pass over any assessments that require taking off cloths until he is more established and comfortable in the new environment. He also has a hard time watching other people eat aversive foods. He will gag. He knows how much he hates to throw up (requiring a bath and a change of cloths) so he trys to hold it back. When this happens we immediately remove him from the situation, and remind him to take deep breaths and think of something else, we also offer water. He will often ask to to wipe his nose after; I think maybe some gets into his nose. He is also sensitive to tactile input, when he was receiving OT weekly we had over come many of his sensitivities, but since Winter he has become much more sensitive again.
