Give me the update please?

I have W.I.C appointment on monday and I thought I had all my paperwork.Well I had to put off my first appointment becasue I was terribly sick. I made a new one adn the lady never send me a damn envelope with the paperwork in it! So I thought the last one I had was good! Well, I went on the W.I.C website today and it says theres now new requirements and paperwork for me to fill out? Wtf? And it started on october 1st. Why would she not send me one? I cant keep rescheduling my appointment, I desperatly need the help with food. Can you ladies tell me all the papers you had to fill out recently if you just recently got on W.I.C. I am so stressed about this  

do you have the number to call them? usually the wic is only the babys doctor info and maybe your income and who you live with 

Pay check stubs, proof of address, ID, baby soc security number, shot records.

check stubs from within 30 days or last one if unemployed brith certificate for all that are applying (i may be wrong on that one...may just be the babys)along with your ID babies social sectiry card registration in other government programs proof of residency (rent receipt utility bill or letter of residency) and shot record

It says I need this new pediatric form?

Did any of you have to fill this out? I had to fill out a breastfeeding/postpartum question paper, babynutrition question paper. I had to have my doctor sign a postpartum paper. I have to take my LO with me, my I.D, proof of address, pay check stub, medical-social security card for LO, her crib card from hospital, and immunization records... But now I need that other paper that I will Hhave NO TIME to get signed by her doctor  

Do you think if I go in without the stupid paper and fill them in on why I dont have it, they'll let me slide? Or just give me another month and a half to wait for another appointment?

I didn't have to do that. See if they will fax it over there for you

My wonderful SO printed out the paper at his work and took it to her doctors right after   Thay said they'll have it in the morning ready for me monday before I go to my appointment! Thank god!  

Oh wow that is ridiculous. All we have ever needed for mine is proof of ID for everyone in the house, income verification (pay stubs), proof of residence, and I think that was really it. Oh and the one paper they send out.
