im finally filing for child support yay me!
i dont have his exact address anymore or a number..
i have some information on him and have his grandmas address and his brothers
cell number..anyways on the back of the paper it says directions to home and absent parent/remarks:

what do i put?

My sis knows about all that ... well call dcf and ask, or just put his name and last known address...

Anything you know put it down. I had the same thing with my SD. He had no Phone and the last known address was the apartment we shared before he kicked me out for getting pregnant. But in the remarks i said "XXXXX is his mom's address and phone number, XXXXX is his dad's address. He used to work for X company, His mom works for wal*mart, this is her name ______, Ect."

what ever you know just tell them.

Oh ya and tattoo's Piercings and any Major scars help identify him. Like if he gets pulled over they will take him down town to fill out his part of the childs support subpoena.
