And your fiancee is really good with him, that is awesome that he has a good father figure even if his bio dad isn't around.
As far as the name change, is your fiancee intending to adopt him officially?  If that is the case, then yes I think it would be great to have his name changed.  If your ex still has any parental rights though I think it can be more difficult, depending if he puts up a fight or not.
