According to dictionary.com "drug" also has this definition:
3.a habit-forming medicinal or illicit substance, esp. a narcotic.
I think that the people who say "I don't do drugs" are referring to that definition. Or more generally, something that you could get fired for if it shows up in your pee and you use it for reasons not prescribed by the doc.
One of the hardest things about learning English, at least in my experience as an ESL teacher, is getting over the fact that a word can have a whole lot of different meanings, the nuance thing, and also how people use general terms to refer to the specific. We say things like, "I locked my keys in the car" when we drive a crossover SUV or "We have two computers at our house" when we live in a condo. In Texas we say, "I need to quit drinking so many Cokes" when all we ever drink is Dr Pepper or Mountain Dew.
It's all acceptable. I think most people know what we mean.Now if you WANT to get more specific for example if you are Mormon and you don't even drink caffeine, or if you are a Christian Science follower and refuse medications, that's the time to get specific. But if I say "I don't do drugs" and someone looks at me blankly and says "Then how are you going to have surgery without anesthesia" I know they either are four years old or are being an ass on purpose.

Exactly, i mean if someone asks me if i do drugs, am i to say... "well, i dont do illegal drugs, but i have caffine, i smoke ciggerettes, i occasionally take paracetamol or asprin, i take anti-bioics and penicilin if i'm ill, i eat sugar, i drink alcohol, etc etc...." i think we would all know what was meant if someone talked about "taking drugs" as if you take anything prescribed or not illegal , then you wouldn't say to someone "i do drugs, or i take drugs" would you?
it's a bit of a pointless discussion in my opinion.
