Aww hun, I'm sorry that you're going through that. I have definatley been there, and don't care to go back. If I were you I would just make a doc's appointment for him and say, you and I are going to the doctors and we are going to get this figured out before it ruins our marriage. Don't take no for an answer, and stick to you guns! You do not have to put up with his behaviour!
