Changing child's last name?

How do you go about a name change for a child? Seeing as his father refused to sign the birth certificate and rarely ever sees him even though we tell him often that he can whenever, I don't want my son to have his name. I'd like it changed to my last name but I'm not sure how to do it.

If he has dads last name I believe you would have to go to court and dad needs to agree and allow him to have your name.

Go to vital records and change it. If the father didn't sign the birth certificate then there shouldn't be a problem changing it. Here you have 1 free name change up until they're a year old.

If the father didn't sign the birth certificate, if there is no custody agreement, and there is no child support order, then you just have to go to vital records.

You go down to vital records (here its in our local health department). Normally you'll need an appt to do name changes. Google "Vital Records in (insert city here)" and call them.
