Daughter acting up in school

My daughter, Adrianna, is 5 years old. She goes to pre-school Monday-Friday for 2 1/2 hours. This morning when her dad went to pick her up from school her teacher pulled him aside and said that Adrianna has been acting up in class, and keeps acting like she doesn't know any answers to what the teachers are asking her. Two weeks ago I got her report card and her teacher told me Adrianna can only count to 39. I have been over numbers with my daughter repeatedly almost every day. and she can count to 100. I hear her for myself. She saying she doesn't know her address, which also I have heard her say repeatedly. I am at work now so I have been trying to think of how I can fix this situation and talk to her when I get home. She already has had her tv time cut out since the report card incident. I'm not sure what else I should do. I don't know what area I am neglecting. Any advice? TIA
