Oh gosh, that's rough!
How about some music... if not to calm him than to at least drown him out for a little bit?
Can you stop somewhere, take him out of his carseat, hold him & just soothe him for a little bit? Maybe get something to eat?
And I thought a 26hr drive from Boston to Kansas with my three cats howling in the background was bad..... sheesh!

Is he normal like that in the car? You might not have a chose you may have to stop. I would try food, give him snacks, sing to him, give him a book to flip, ummm sit in back seat with him

Sit in back with him like I do with my son. I bring lots of toys and books and I'm breastfeeding him still, he's 18 months, and if he's THAT fussy ill lean over the seat an nurse him. I'd go with what the last person said too and get out at a rest stop and let him run around to release his energy, he may just be over tired and is ma he's not in his own bed.. My toddlers watched maybe 39 minutes all together of TV his entire life but we bought a DVD player before our vacation last month KNOWing that we'd need SOMEthing. And sure enough it helped like a damn sedative. He was so entertained because he's never sat there and watched any shows before so the 14 hour trip wasn't bad at all. Elmo's world and Barney! I know this may not hell now but for future references, we got a used one from a second hand video:game/music store and it was $25 and ended up being brand new! The DVD player still had the cushiony thing on the inside.

Ok ill describe it the best I can because I read it in a book when my 18 month old son was still an infant, after we had three straight nights of crying and nothing working to soothe him, I found this book and the 5 S's worked magic I swear. The book is called The Happiest Baby in the Block. The five S's remind baby of being in the womb, which was the most peaceful and calming place of baby's life.. And it doesn't matter the age, jasper grew out of needing the 5 S's by like 9 months but we used the swaddle til 15 months wen he just decide he didnt want it anymore.
1. Swaddle.
2. Side/stomach position.
3. Swing/sway.
4.sssshhh (making shushing noises an inch away from baby's ear, reminds him of the vacuum noises heard in utero)
5.suck (pacifier or breast)
Do all those things together in that order and consistently and within 5 minutes hell be done crying, but don't stop doing them when he gets quieter, keep doing them consistently til he's completely done crying.
