Let's make it worse.

The 4 guys in the cave are a doctor, a doula, a midwife and the pregnant woman's husband. They all went in to have a birth with nature. As the pregnant woman was screaming out in labor pains, the cave entrance collapsed. The husband and the doctor, trying to be life-savers, pushed the laboring pregnant woman into the entrance to try and get her out first. She got stuck. The baby is crowning and they expect it'll be born in a few minutes. High tide is in 5 minutes. It will take her 4 minutes to get out and safely deliver the baby, leaving less than 1 minute for the rest to get out. They decide to use the dynamite to blow up the mother and partially born baby. The cave collapses on them and they are killed upon impact. The tide comes and washes away the rocks, exposing the dead bodies. A few hikers (3 hikers) who had heard the screaming and explosion come to inspect the scene. They see the dead bodies and immediately start vomiting all over each other. One of the hikers, also a pregnant woman, slips on the vomit and falls. The two men see a small ledge that one of them can climb down on and push her back up. The two have a new dilemma, who climbs down to push her back up? Hiker A is her husband. Hiker B is her brother. Whoever climbs down risks falling to their untimely death into the pool of dead bodies.
