I need help!

Ok, I'll explain this as easy as I can.

I receive a Functional Ability Evaluation Medical Report from the DMV each year. I have my doctor fill it out and turn it in to them. Well although I haven't received the report yet I am worried that I won't be able to get it filled out due to me not having insurance.

What should I do?

Speaking of insurance, how can I get insurance? Medicaid denied me 3 times now when I was eligible for it. If any of you can tell me what I need to do to get affordable insurance that would be great.
