Did you make one yourself or print one? What does yours look like?
Is it a sticker chart?

His psychologist wants me to keep an hourly log of what he's doing and a behavioral chart.

My mum has one for my little brother, its like a white board one, you can print them off or buy them yourself, even wright them out yourself.

just made squares and on the bottom made a ledger... so many stickers he got something... if he filled the whole board he got chuck e cheese and if he was bad i take a sticker off or put an x over it and hed have to redo that sticker..

I made my own. We have a get ready chart for in the morning and a "get off the playground right away" chart for when I pick him up from school that we keep right in the car. He puts a sticker on when he gets in the car if he comes right away. We never take stickers off after he has earned them. However, if he doesn't get off the playground right away he doesn't get a sticker for the day. Your chart doesn't need to be anything fancy, just talk it up so it's super exciting!

We use the Melissa & Doug Responsibility Chart. It has everything from chores to behaviors. I love that it's all magnets & I'm able to easily switch their goals week to week. It also has a white board on the bottom, so I can write personal notes, words of encouragement or reminders...etc.
