Filing for custody?

I decided to file for custody for LO, considering me and BD don't have a custody agreement in place. I'm filing for full custody, no visitation. BD doesn't care to see LO, has seen him once by choice in two years. I'm not changing my mind, this is what I know is best.
I was told by a friend that if I could find the paperwork, I could file myself for around $100. My lawyer will do it for $1,500. Right now, we're waiting to see how much money we're going to have to spend, which will be our final deciding factor. I'd prefer to go the lawyer route, but if I don't have the money to spend, I have to file myself because we really need to get a legal custody agreement in place. I have a friend that's interested in filing for legal custody herself, so I was wondering.
Can anyone possibly help me find the papers? Even if I don't use them, my friend will for her 4 year old son, so I told her I'd try to find them. My other friend said she'd let me use hers and just scan it, wipe out her info and use my own, but she appears to have lost her papers. We both live in Surry county, NC if that helps...(I tried to google it, but only found papers for Mecklenburg county)

They should be on your County courthouse site. You can call the courthouse as well.

They're not very helpful at that particular court house, lol...
I'm checking out the website now though, maybe I can find something.

ETA: I'm looking, but I don't see anything regarding any kind of legal forms. I tried to use the search function, and it keeps throwing me to the front page again.

I looked through the link you posted and found the forms area. At the top of the page click on where it says citizens. Then there is a column on the left click on going to court. Next there is another column on the left labeled forms. I hope that helps.

I went through and looked but didn't see anything. However, I don't have my glasses and I'm also at a friend's house lol, so I'm having her to check.
