These guys appeared on dragons den(Canada's version of shark tank) I thought it was useless but apparently it could be handy to some people.

Not sure what you call large but my testicles are about grape sized and they hurt occasionally during sex too - to the point that missionary or doggy is painful. Testicles can vary in size at various times, temperature too, and they also become sensitive at certain times. Some days we might like it rough, other days cant bare to be touched.

I have absolutely no useful suggestions but I just had to let you know your solution to the problem literally made me LOL.  

He can just leave his underwear on and put his penis though the fly. That should give his testicles some support.

Try the boxer/briefs with the hole and just have him pull it out lol
