Personalized birth plans

Does anyone have a copy of theirs that I can look at?
I don't want one off thebump.com woth boxes to check and such, my OB has requested mine be done by my next appointment and I feel a bit lost.

Basically I want my birth to be a home birth in a hospital setting, I will labor at home as long as possible. I do not want pain meds even mentioned in my room. No episiotomy. Want to be free to move around my room. (Will have CFM because of VBAC status so will not get to leave room). Free to drink liquids, no catheter. Mother directed pushing in any position I feel comfortable. SO to catch baby, immediate skin to skin contact, and to wait until cord stops pulsing to cut. SO cuts cord. Breastfeeding exclusively, no botttles or pacis. Baby shall not be taken out of room and all assestments done bedside in my presence. I will give him his first bath. No circumcision.

Am I missing anything?

I think I'm ok with the Vitamin K shot but haven't decided on the eye stuff yet.

I look up those premade birth plans then just write down which ones are relevant to me and my birth plan that way I'm not missing anything

Thats what I'mdoing I just feel like Im missing something.
