Vaccine exemption forms for school

Does anyone know where to find the forms for vaccine exemption? My son starts school this year and I want it taken care of. Also do the forms differ by state?

The school should have one.

The links that I have found, come up saying "page cannot be found"
I did see that it is by state though

I got mine at the health department.

Don't fill out any forms you get from a school, Doctor, etc. The wording is meant to implicate you in the case of illness.
Always use these.

She's asking for the exemption form, not the crap the doctor tries to get you to sign at your appointments.
