I've been in a similar situation, I told the girl, and now she's about to get married to someone else and we're very good friends.

I would end the relationship, and give him an opportunity to tell her. If he doesn't, I will; she has a right to know.

I was the other woman back in 2007. I had no idea, and it only lasted like 3-4 months. I was out having lunch with my friend and I saw him there with his fiance and their 2 children. He saw me too and he was terrified. I didn't say anything then because their children were present. Later that night he called me and told me they had never split up like he told me and he was sorry for lying to me. I told him if he ever contacted me again I would tell her everything, and I had pictures to prove our relationship. I never heard from him again.

I would tell her, right after I broke it off with him. If I were the girlfriend/wife, I would want to know.
It wouldn't matter to me if she was a girlfriend/wife, and them having kids would make me more inclined to tell.

Of course I would tell her, because I would want to know if I were her.
No, because regardless, she's being lied to, and that just doesn't sit right with me.
Same as above, it's like he's lying to the kids, too.
