Pregnant , working and maternity leave.

I'm due with my second on Feb 14th, 2013.
I'm still working (even though it's been tough on me) but I want to work as long as I can.
I want to stop working around 38 weeks, though.
(I had my son at 38 weeks) so I'm thinking of telling my employment I'd like my last day to be Jan 29th or 30th.
Should I get a note from the doctor first? (when it gets closer) or how do I go about doing this?
Also, if you are/were working while pregnant when did you take off of work?

I worked full-time in a nursing home until I was 38 weeks pregnant. Only reason I stopped working when I did was because my blood pressure was starting to creep up and I swollen up like the Michelin Man.

I called my boss at 37w3d and was like, yeah I'm 3cm and not supposed to work anymore, and he was fine with that.
But i worked as a hostess at a restaurant, you probably have a real job lol

I've already had a lot of complications so far this pregnancy.. and the doctor is already thinking I will go before my due date. Esp since I went at 38 weeks with my son, but I know every pregnancy is different.

I plan on starting leave exactly 2 weeks before my due date. I had my son 10 days early so I figured it I have my daughter the same as my son was I won't have to worry about going into labor at work. Where I work we are by ourselves on almost every shift and I just didn't want to be stuck there by myself and go into labor. Unless something else comes up I plan on working that long.
