Vaccination Form

From school. I'm being a post whore for a minute.
I got sent home with this paper: Washington Sate Requires any child that visits a public school building be updated on all of their immunizations. Because your child visits out school to receive speech services, this law applies to your family.
The attached forms are requested to be filled out and returned as soon as possible.
PLEASE RETURN THESE FORMS TO YOUR SPEECH AND LANGUAGE PATHOLOGIST AT YOUR NEXT SPEECH SESSION!

But according to our pediatrician he just needs to sign a waiver. So why do they send things like ^^^^ that home with me? And, I don't think he's up to date. Neither of my other kids are for sure. And we aren't giving them any more. So should I call them and tell them we don't vaccinate and ask what to do? Schedule and appt with our ped? Thanks!

There should be a waiver form you get from the actual government there's info on how to actually get it on that website for vaccines lol I don't know what it is, but you can google it or hopefully some helpful mamas on here will post it for you.

I think that's the form my ped. will provide. ?

Possiblyyyyy? I'm not completely sure on this issue bc LO isn't here yet and I just don't plan to vaccinate so I just remember seeing when I was researching everything to come to the conclusion that there's a form you get for schools.
Calling the health department is a good idea, too. They'll for sure know how to get ahold of it.
