Immunization Exempt for NY

Anyone know a immunization exempt form I can print out? All the websites I found for NY say I have to request a form with thorough explanation.

Look for your state and the forms should be there. Just fill out and take them to your baby's pediatric. It says a lot of NY parents have had their forms denied but, mainly approved if your religious.

I havnt had my boys immunised and i didnt ave to fill in a form but i am in britain... i didnt realise you had to have it done in america it shocked what happens if you dont tke your baby to be immunised?
