i am not sure about the states, but here you have to go to the clinic, go to mental and they do marriage counseling up there.
it can take 6+ weeks to get an appointment here though.  

Military one source!
Just call & they find a provider in your area and set up your first appointment!

You need to visit your family readiness center. What branch? You can also call military one source and they will give you a referral to a counselor in your area if you don't want to see one on post. If all else fails, call your chaplain.

Is it completely covered by insurance?

I'm not sure how it works for couples, but through MilitaryOneSource you can get 8 sessions free/without referral with them. Otherwise, try getting in touch with the Chaplain and see if they can get you started.
