Academic Progress Appeal Form?

Can anyone help me fill this out?
This is the only question I'm having trouble with.
Explain what has changed that will allow you to meet the Satisfactory Academic Progress Policy at the next evaluation. Outline your detailed plan to achieve academic success at Kent State University and include the specifics steps you have taken to meet this plan.

I just want to answer it like, "Um, I'm not pregnant anymore.."

I dropped out of college in 2010. Classes started in August, I found out I was pregnant in September. I was 17, I was failing my morning class & kept changing my major.  

"I have established a support system to help me manage my time more effectively. This system will ensure my success in my academics by motivating me, managing time effectively, and allowing me ample opportunity to focus on my studies."

I worked it into my short essay of PLEASE GIVE ME ANOTHER CHANCE!!! lol
Hopefully it's not too late to get my financial aid. I just had my classes scheduled today and no way to pay for them yet.
