I always thought that too. But apparently (I don't remember how this conversation started) one of my exes told me that they NEVER use that thing to pee out of and that they just pull the front of their boxers out and whip it out cuz its quicker. Idk. LOL

DH uses the hole when he's not home or using urinals but, at home he just pulls his whole situation out, balls and all and pees like that. I remember walking in to brush my teeth and I was like, "Why are your balls out?" and he said, "It flows better when the waist band isn't pushing into it."

Also I like wearing guys boxers to bed so I inquired about this to two exes and they said it was for this too.
